Definition of GERD
GERD (Gastro-esophageal-reflux-disease) is a term to define to any symptomatic clinical condition resulting from episode of Reflux of acid, pepsin and occasionally bile into the esophagus from the stomach tract
heart-burn and problem in swallowing. 🙁
Complications include esophageal structural changing because of formation of specialized protective cellular line present at the stomach-esophageal junction known as Barrett esophagus ,the mechanism of Acid-Reflux is multifactorial and involve transient lower esophageal sphincter relaxation-contraction cycle, decrease tone of the lower esophageal sphincter will affect the severity of symptoms by disturbing normal body function
Hypersensitivity to normal acid exposure may be the cause of complications, progression from non-erosive-Reflux disease to erosive esophagitis is rare!
Management of GERD
It focus on symptomatic treatment.
By endoscopic findings careful symptoms evaluation is required for treatment in patients presented with alarming symptoms, and those who fail to respond to medical treatment should be referred to endoscopic investigation and detection test for H.pylori.
It includes Lifestyle modifications such as weight loss and smoking cessation in combination with antacid and these measures may be effective in patient with no much impairment of quality of life equally.
When quality of life is impaired acid suppression therapy is the basis of the effective treatment course, standard dose of PPI therapy is most effective for symptoms, healing of esophagitis and maintenance of normal body functions in patient with GERD.
